• Submit an Identity Theft Affidavit claim 14039
• Notify your financial institutions
• Check your credit report with one of the three major bureaus:

File a complaint with the Federal Trade Commission using one of the following:

• By telephone at 877-438-4338
• By mail at:

  Federal Trade Commission
  600 Pennsylvania Avenue NW
  Washington, DC 20580
• Contact your local Social Security Administration office at 800-772-1213 or online at SSA.gov to ensure your earning records are accurate.
• Visit IRS.gov/IdentityTheft for more information on tax related identity theft.
